The Patient Care Coordinator is responsible for managing various aspects of patient care.

The intake process, ensuring a smooth and efficient communication and flow of information while providing excellent customer service.

Oversight of daily operations of home health aide services such as supervision of home health aides, ensuring Electronic Visit Verification (EVV) compliance, initiating discharges, scheduling / coordination of home health aide services and collaborating with clinicians and families to ensure that clients receive timely and appropriate care in their homes.

• Intake :   Address patient inquiries and concerns with empathy and professionalism.
• Provide clear information regarding services and the intake process, including required documentation and procedures.
• Collect and verify patient information, including medical history, demographic details, and insurance / payment information.
• Enter patient information accurately and in a timely manner into electronic health records (EHR) systems.
• Collaborate closely with nursing staff and administrative teams to ensure an efficient intake process and start of services.
• Identify and resolve issues related to patient intake, such as discrepancies in information or scheduling conflicts.
• Provide support to patients in navigating the healthcare system.
• Maintain patient confidentiality and adhere to HIPAA regulations.
