Job Summary : The Behavioral Health Consultant is a core member of the collaborative care team, including the patient's medical provider, mental health consultants, as well as the larger primary care team, and the Comprehensive Care Coordination Department. The Behavioral Health Consultant is responsible for providing direct patient care, supporting and coordinating the mental and physical health care of referred patients with the patient's medical provider, and when appropriate, mental health providers.
