• Provide emotional and social support to residents and families
• Oversee admissions, discharge planning, and care transitions
• Advocate for residents’ rights and well-being
• Ensure compliance with state and federal regulations
• Work closely with interdisciplinary teams to enhance resident care

Requirements:

• Experience in social services in a long-term care setting
• Knowledge of Medicare, Medicaid, and discharge planning
• Strong communication and problem-solving skills
• Bachelor's degree in Social Work or related field (preferred)
