POSITION SUMMARY :

In this contractor role, the MD will be responsible for establishing the Tapestry Health patient relationship for our chronic care management services.

KEY RESPONSIBILITIES :

Responsible for establishing the Tapestry Health patient relationship for our chronic care management services.

Be available at a minimum of 12 working days per month to conduct approximately 375 clinical visits, both remotely and onsite. Travel is typically 1 hour from home and we pay mileage.

Document patient encounters accurately and thoroughly within our platform at time of visit.

Provide end of day report to program administrators to track progress for completion of visits.

MINIMUM REQUIREMENTS :

Qualified MD / DO

Active, unrestricted, Medical Licenses in Texas and at least 10 other states.

Excellent clinical judgment and decision-making skills.

Must have reliable transportation to travel to assigned buildings.

Commitment to providing patient centered care and improving health outcomes.

Familiarity with electronic health record (EHR) systems and healthcare technology.

Ability to travel within 1-2 hours of assigned region
