Key Responsibilities

Provides support to ambulatory care management programs and conducts patient outreach

Identifies and manages patients' care needs according to established protocols

Assesses barriers to care and provides coordination and assistance to patients

Required Qualifications

High School Diploma or GED

2-5 years of relevant experience

Licensed Practical Nurse or Registered Medical Assistant certification
