Registered Nurse Care Transition

A company is looking for a Care Transition Nurse (RN) - Population Health.

Key Responsibilities

Identify, enroll, and manage patients transitioning from acute care to community settings

Develop and implement transition care plans to enhance healthcare outcomes and ensure safe discharge

Collaborate with healthcare teams to execute patient-centered care plans and track referrals to resources

Required Qualifications

Associate's Degree in Nursing; Bachelor's Degree (BSN) preferred

Active Registered Nurse license in the state of patient care

2-3 years of experience in acute care, home health, or case management

Case Management certification preferred

Knowledge of patient care across various age groups
