Case Manager Homecare RN

Must have experience with Certified Home Health Agencies (CHHA)

RESPONSIBILITIES - not limited to :

complete head to toe assessment.

at-home IV infusion, wound assessment, wound care, and education aimed at

promoting and maintaining patient independence.

case manager, patient, family, physician, care delivery team, and third party payors

including obtaining medical orders and approval of medical services.

disease process, self-care techniques and preventive strategies.

QUALIFICATIONS :

1. New York State Licensed RN

2. One year of experience as a clinical nurse in a hospital or other direct patient care

clinical setting required

3. Strong critical thinking skills, the ability to follow standards / procedures, and make

appropriate clinical judgments to provide safe and effective client care

4. Ability to work independently with minimal supervision, adapt to change, and

manage a variety of assignments while establishing priorities and ensuring the

necessary duties are completed

5. Strong interviewing, clinical assessment and communication skills

6. Ability to exercise initiative and independent judgment

7. Ability to build and maintain professional relationships

8. Current NYS driver’s license
