A company is looking for a Care Coordinator to serve as a patient advocate and point of contact for care recommendations.
Key Responsibilities
Support patient experience preadmission and throughout the continuum of care
Ensure timely follow-up with patients and families, maintaining open communication
Facilitate internal transfers between sites and develop working relationships with interdisciplinary team members
Required Qualifications, Training, and Education
High School diploma required; Bachelor's degree preferred
Two years of customer service and relevant experience, preferably in a healthcare setting
Experience with computers and proficiency in software platforms and EMRs
Knowledge of medical / psychiatric / chemical dependency diagnosis and treatment processes preferred
Ability to work rotating on-call weekend and holiday shifts
