First Connect Health is seeking a travel nurse RN Care Manager for a travel nursing job in Jackson, Mississippi. Job Description & Requirements Specialty : Care Manager Discipline : RN Duration : 13 weeks 39 hours per week Shift : 8 hours, days Employment Type : Travel RN Case Manager – Discharge Planning (Direct Hire) Location : University of Maryland Medical Center (UMMC) Schedule : Monday–Friday, 8 : 00 AM – 4 : 30 PM (5x8s, non-negotiable) Department : Case Management – Discharge Planning Requirements : RN Case Manager with 2 years of Acute Care Management experience in discharge planning. Minimum 6 months at a Level 1 Trauma or large Academic Medical Center handling varied inpatient units. EPIC charting experience required. BLS required; CCM preferred but not mandatory. BSN preferred , will consider ADN . Role Overview : Manage 15–20 cases daily , focusing on inpatient discharge planning. Work with a multidisciplinary team including 15 full-time and 17 part-time Social Workers. Handle medical and social discharges, coordinating placements with contracted local facilities. Collaborate with RT and Pharmacy teams for discharge needs. Report directly to the Social Work Manager ; department oversight by Chief of Care Coordination . Additional Details : No weekends required. Holiday rotation as needed. Dress code : Any color scrubs or business professional attire. Orientation : 2 weeks (includes general facility orientation, mentorship alongside CM team). EMR : Using naviHealth; transitioning to EPIC Care Management in the coming year. General understanding of discharge readiness required (no InterQual / Milliman needed).
