Job Description
Winston is hiring licensed social workers (LMSW or LCSW) for a 3-month long contract positions in hospital settings in New York City.
Responsibilities :
- Provide services to patients and their families including screening, patient assessment
- Assists patients in continuity of care planning and in enhancing their capabilities of coping with illness and daily functioning
- Provides discharge planning services for post-acute needs
- Provide limited counseling, crisis intervention, and advocacy
- Collaborates with the interdisciplinary team to assess and plan for appropriate care and services
- Uses knowledge of community resources to refer patients to appropriate services to ensure continuity and quality of care
Minimum Qualifications :
- Hospital discharge planning experience
- Master’s Degree in Social Work
- Current NYS Social Work License (LMSW, LCSW preferred)
