Hospice Licensed Vocational Nurse (LVN) - Per Diem

We are seeking a full-time LVN Licensed Vocational Nurse to join our team! You will be responsible for delivering high-quality care to assigned Hospice patients. Clinical experience is preferred and candidates with Hospice experience are highly desirable.




​Responsibilities:

• Provide Skilled Nursing Care in a home or facility setting
• Ensure the appropriate use of meds, DME, and supplies
• Educate and support patients, families, and caregivers
• Participate in IDT and updating the Plan of Care
• Provide our patients and families the best care possible
• Daily travel is required when working

​Qualifications:

• Valid LVN License in California
• One year or more of experience in either Home Health or Hospice is preferred, but not required
• Previous experience in a hospice setting is a plus
• Ability to build rapport with patients
• Compassionate and caring demeanor
• Excellent written and verbal communication skills
