Referral Services Administrative RN
Partners In Care is seeking a Referral Services RN responsible for all administrative aspects of the patient intake process, establishing and maintaining positive relationships with customers and referral sources, and responding to customer requests and concerns.
Collaborating with an interdisciplinary team of social workers, RN case managers, volunteers, chaplains, and more to enhance the lives of our patients and their families.
Compensation : $40.00-$55.63 (DOE).
Location : Bend, Oregon.
Schedule : Full-time, Monday- Friday, 8 am-5 pm.

What You Get To Do :
- Assist in strategic planning, including identifying opportunities for additional or improved services to meet customer needs.
- Maintain comprehensive working knowledge of community resources and assist referral sources in accessing community resources should services not be provided by Partners in Care.
- Assist with implementing improved work methods and procedures to ensure patients are admitted per policy. Does not direct but follows the implementation.
- Complete referral worksheets for all services and request all needed information from referral sources.
- Faxe requested CTI from the physician and obtained the physician's signature.
- Enter ROC order in Electronic Medical Record (EMR).
- Assist with incoming calls and takes referrals.
- Notify the Nurse Practitioner and MD team if a hospice patient is in the 3rd or greater benefit period needing pre-admit F2F.
- Establish and maintain positive working relationships with current and potential referral sources.
- Ensure compliance with all state, federal, and referral intake regulatory requirements.
- Check Medicare insurance for Hospice referrals when administrative staff is not available. If the patient is in the second or greater benefit period, enter information into the electronic medical record (EMR) to generate the correct certification date.
- Check Medicaid eligibility for hospice.
- Adhere to all policies, procedures, and regulations regarding safe, ethical conduct, security, and risk management.
Required Skills & Qualifications :
- Graduate of an accredited school of nursing with current licensure to practice professional nursing in the State of Oregon.
- At least two years experience in health care.
- Excellent assessment, problem-solving skills, and nursing skills per competency checklist.
- Strong clinical judgment and patient triaging skills.
- Efficient time management with minimal oversight.
- Proficiency with Adobe Acrobat, Microsoft Word, and Microsoft Outlook.
- Ability to learn and effectively utilize a large array of different computer programs and platforms.
- Ability to navigate between multiple software systems and programs concurrently, often while on the phone.
- Positive phone interaction and customer service.
- Ability to pass a background test, drug screen, and TB test.
- Ideal candidates will possess 1-2 years of home health or hospice experience.
- Valid CPR Certification.
- Current automobile insurance, a valid Oregon driver’s license, and reliable transportation are required for those working in the field.
- Ability to work independently and collaboratively within an interdisciplinary team.
