St. Paul’s PACE is seeking a Primary Care Physician to join a Program of All-Inclusive Care for the Elderly in San Diego. The program serves individuals 55 years or older living with chronic illness or disabilities who need coordinated medical care to remain independent at home and in the community. The healthcare teams provide hands-on coordination of medical, social, and home care services.

Responsibilities
St. Paul’s PACE is seeking a Primary Care Physician that can :
Coordinate with the interdisciplinary team to develop a comprehensive care plan for each participantIntegrate the primary care treatment plan into the overall plan of care developed by the interdisciplinary team; interact with team members to meet emergent and acute needs of participantsCreate a welcoming environment for all St. Paul’s PACE participants and develop inspiring, lasting relationshipsKey Responsibilities
As a Primary Care Physician at St. Paul’s PACE, you will :
Deliver high-quality, comprehensive primary care to frail older adults in coordination with an interdisciplinary teamPerform initial, semi-annual, and ongoing assessments to develop individualized care plansIntegrate the primary care treatment plan into the broader plan of care developed by the interdisciplinary teamEvaluate and manage chronic conditions and acute illnessesProvide preventive care, including immunizations, screenings, and health maintenanceParticipate in care planning and discharge decisions for hospitalizations or long-term placementCommunicate effectively with nursing staff, specialists, and other providers to ensure seamless, coordinated careMake home, assisted living, and nursing facility visits as neededProvide ongoing clinical support and training to staffParticipate in team meetings, peer review, quality improvement, and compliance effortsContribute to clinical policy development and program evaluation under the guidance of the Medical DirectorEducation
MD / DO with interest in geriatrics. Board certified or Board eligible (if newly graduated from residency) in Internal Medicine, Family Practice, or Geriatric Medicine desired. Current state license and DEA registration required. Meets requirements of 42 CFR Part 410.20.
Experience
A minimum of one year experience working with a frail or elderly population is required. Shall have either training or related experience in the assigned job.
