Job Description
Responsibilities Responsible for being able to perform all aspects of the hospital’s assessment and referral process, including efficient and effective handling of inquiry calls, knowledge of and communication with referral sources; providing hospital and program information to individuals, agencies, facilities, and professionals; making outside referrals, when appropriate; assessing and triaging an individual’s mental health and/or chemical dependency status; making patient safety and level of care recommendations to the physicians(s); identifying insurance benefits, pre-certification of patient care; completing all admission requirements; communicating patient safety concerns and pertinent patient information to unit nursing staff and documenting all necessary information.

Qualifications
Minimum Qualifications
Masters Degree in from an accredited college or university in social work, counseling or other related fields or Nursing Degree required
Minimum of two (2) years experience in the mental health field preferred.
Excellent communication skills. Ability to relate to those psychiatric and chemical dependency issues and in crisis.
Medium demand, mentally. Requires extensive time sitting, writing, and talking on telephone. Ability to assist in crisis intervention, physical intervention with out of control patients.
Performs others duties as may be assigned by Director of Assessment and Referral Services; ADARS; and CEO.
