Experience conducting comprehensive assessments of patients.
Description :
Required Licensure : Current RN Licensure in Client.
Required Experience : Two years of experience in a home setting within the last ten years.
Nurses must have recent experience conducting comprehensive assessments of patients.
All nurse assessors must meet specific requirements.
Experience can include working for a certified home health agency, licensed home care service agency, or managed long-term care.
Conducting assessments similar to UAS such as OASIS is necessary.
Providing primary care to homebound persons is required.
Care management for persons receiving home and community-based services, including hospice and assisted living.
Home care experience as a paraprofessional or licensed practical nurse, caring for a minimum of two patients per week.
Preferred Experience :
Experience conducting assessments in UAS-NY, especially in the last 365 days.
Experience caring for a geriatric population and managing chronic diseases in home settings.
Combination of home care experience as a paraprofessional or LPN alongside RN experience.
Other experience examples may count towards preferred qualifications but do not satisfy required qualifications on their own.
Ability to review health status questions and make evaluations based on UAS criteria.
Ability to work with technology in the workplace.
Knowledge of the theory, principles, and practices of general professional nursing.
Understanding of state laws and regulations governing professional nursing practices.
Excellent organizational, interpersonal, written, and verbal communication skills.
Ability to communicate effectively with multi-generational consumers.
Demonstrates sensitivity to the concerns of others.
Ability to perform in a fast-paced, deadline-oriented work environment.
Capacity to manage multiple complex tasks simultaneously.
Strong ability to work independently.
Perform initial in-home evaluations of patients.
Evaluate health status, strengths, care needs, and preferences.
Guide the development of individualized long-term care service plans.
Review consumer medical documentation and relevant health referral forms.
Enter evaluation data into electronic forms and transmit as required.
Document any concerns or issues during the evaluation process.
Emphasize continuity of care to reduce fragmentation and gaps in treatment.
Discuss healthcare options and supports needed with consumers.
Initiate communication for enrollment into managed care health plans.
Enhance communication and collaboration with interdisciplinary care team members.
Collect quality review data and required documentation for outcome measurements.
Identify opportunities for health promotion and illness prevention.
Maintain comprehensive knowledge of community resources and services for targeted populations.
Meet all standards for performance as outlined in established criteria for the position.
Perform other duties as assigned by the Regional Nurse Manager or project management.
