Southside Community Health Services (SCHS), a trusted nonprofit Federally Qualified Health Center in the heart of South Minneapolis, is seeking a mission-driven Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our growing primary care team.

About the Role
The Advanced Practice Provider (NP / PA) provides exceptional, patient-centered primary care to help patients achieve their highest level of health within a community health setting.
Key Responsibilities
Patient Care
Provide comprehensive, patient- and family-centered care within scope of practice, including health maintenance, preventive exams, acute and episodic care, chronic disease management, and patient education.Collaborate with an interdisciplinary team to manage care, coordinate services, and support patients in achieving their health goals.Complete full patient contact hours based on FTE.Participate in an on-call schedule as needed, based on clinic needs and provider group structure.Administrative
Complete disability, FMLA, workers’ compensation, and other necessary forms and letters within two regularly scheduled business days that the provider works.Review timecards to ensure work hours and PTO are accurately recorded.Quality & Compliance
Adhere to timely documentation standards, including closing charts and addressing Epic in-basket messages within 48 hours.Complete peer reviews in a timely manner.Maintain all credentialing, licensure, and certification requirements.Follow SCHS policies and documented clinical protocols.Complete required Relias trainings, policy attestations, incident reports, assignments, and email follow-up in a timely fashion.Other

Education :Advanced Practice Registered Nurse (APRN) or Physician Assistant degree from an accredited institution.Licensure & Certification :Current Minnesota license as an Advanced Practice Registered Nurse or Physician Assistant (PA-C) in good standing, with no pending or previous disciplinary actions.Current DEA license and BLS certification.Experience :Minimum 1 year of experience in a clinic-based or community health center environment required .2+ years of experience in a clinic-based or community health center environment preferred .Technical :Experience with electronic medical records; Epic experience preferred.Language :Bilingual (English / Spanish) strongly preferred.Position Details & Benefits
This is a 0.75–1.0 FTE, salaried, exempt position with a pay range of $128,000–$135,000 annually (based on 1.0 FTE), depending on experience.
Work schedule : 3–4 ten-hour days per week.
This position is eligible for student loan forgiveness.

Work Environment & Physical Demands
This position operates in a professional medical clinic environment and routinely uses standard office and clinical equipment.
Physical demands include :
Operation of typical office equipment (computers, fax machines, copiers, telephones) and medical instruments (diagnostic tools, medical devices).Effective verbal and written communication to interact with team members, patients, and complete documentation.Ability to discern test and imaging results and observe changes in patient condition.Regular interaction with individuals who may have communicable illnesses; potential exposure to infectious diseases.Ability to remain in a stationary position for extended periods and move frequently throughout the clinic.Ability to move items (medical supplies, equipment) up to 10 lbs.Assist patients of varying sizes onto / off exam tables and position oneself to perform necessary tasks.Use of stairs or ladders as needed to retrieve items.Ability to recognize and appropriately respond to signs of health issues or impairment.Join Our Mission-Driven Team
