QUALIFICATIONS :

• Health - Ability to meet performance requirements.
• Education, Experience and Skills :
• Graduate from an approved school of nursing, College or University.
• Must have a current, unencumbered license to practice nursing as a Registered Nurse / Licensed Practical Nurse in Florida.
• Participates in continuing education so that he / she has knowledge of current trends and developments in nursing and maintains current license.
• Minimum of two years of RAI and Care Plan experience, preferably in a Skilled Nursing Facility (SNF).
• Must have knowledge of MDS 3.0 including Assessments, CAA's and Care Plans.
• Conduct each Assessment and Care Plan with the appropriate participation of health professionals.
• Ability to organize work procedures, assume responsibility, and tactfully interact with other disciplines.
• Must be able to communicate well with any member of the Interdisciplinary Team.
• Collaborate with the IDT to provide guidance on appropriate and precise charting for the medical record.
