About the Role
For one of our client in healthcare, which is a mission-driven organization delivering comprehensive, compassionate care to the elderly and medically vulnerable, we are seeking a dedicated and experienced Social Work Manager to lead their social services and behavioral health team.
In this vital leadership role, you will oversee a team of Social Workers, Behavioral Health Specialists, and a Social Work Supervisor—ensuring high-quality care that aligns with each participant’s individualized care plan.
What you'll be doing
- Lead and supervise a multidisciplinary team including Social Workers, Behavioral Health Specialists, and a Social Work Supervisor.
- Oversee the quality and consistency of psychosocial assessments, care coordination, and participation in Interdisciplinary Team (IDT) meetings.
- Ensure timely and effective delivery of social work services in response to participant needs.
- Support professional development and performance of the social work team.
- Collaborate closely with clinical leadership to improve care models and participant outcomes.
Who you are
Minimum 6 years of progressive experience in social work, including at least 2 years in a supervisory or leadership role along with 1 year of experience with a frail or elderly population.Master’s degree in Social Work (MSW) required.Social work licensure preferred.Strong interpersonal and communication skills; ability to lead with empathy and accountability.Valid state driver’s license with a clean DMV record.Copy of recent vehicle insurance required.What We Offer
