Job Summary :
We are currently seeking a compassionate and dedicated Registered Nurse (RN) to join our Hospice Care team as a Day Shift Case Manager.
In this role, you will be responsible for providing comprehensive and holistic care to patients receiving hospice services.
The ideal candidate will have experience in hospice care and a strong commitment to providing comfort and support to patients and their families during end-of-life care.
Responsibilities :
- Conduct initial and ongoing assessments of patients' physical, emotional, and spiritual needs.
- Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team.
- Coordinate patient care and services to ensure continuity and quality of care throughout the hospice journey.
- Provide skilled nursing care, including pain and symptom management, wound care, and medication administration.
- Educate patients and their families about hospice services, disease progression, and end-of-life care options.
- Offer emotional support and counseling to patients and families facing terminal illness.
- Collaborate with physicians, nurse practitioners, social workers, chaplains, and other healthcare professionals to address the holistic needs of patients.
- Document all aspects of patient care accurately and in compliance with regulatory requirements.
- Participate in interdisciplinary team meetings and case conferences to review patient cases and adjust care plans as needed.
Qualifications :
- Valid Registered Nurse (RN) license in the state of practice.
- Minimum of one year of experience in hospice care or palliative care preferred.
- Certification in Hospice and Palliative Nursing (CHPN) or willingness to obtain certification within one year of hire.
- Strong assessment and communication skills.
- Compassionate and empathetic approach to patient care.
- Ability to work effectively in a multidisciplinary team environment.
- Understanding of end-of-life care principles and practices.
