GENERAL PURPOSE
Responsible for assisting members in meeting their expressed goals toward housing and maintaining wellness while living in the community.
Additional support in areas of transportation, counseling and advocacy will be provided as needed.
DISTINGUISHING CHARACTERISTICS
This is an at-will direct service position within a program. The position is responsible for the attainment of housing resources for clients and provides direct assistance when needed.
ESSENTIAL DUTIES AND RESPONSIBILITIES (ILLUSTRATIVE ONLY)

- Works with clients to obtain and keep housing of their choice.
- Develops and maintains a listing of housing options for clients.
- Safely transports members and other passengers to and from appointments and activities using own personal vehicle.
- Attends in-services and training on housing in the community.
- Acts as liaison with board and care operators, room and board operators, county, and staff.
- Keeps all other involved staff and agencies informed of client’s progress.
- Provides support in transporting clients to appointments related to housing.
- Provides documentation in the charts that is consistent with the service plan.
- Provides prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis situation;
provides on-the-spot counseling that is both helpful to the members and consistent with the philosophy of the program.
- Insures the safety, health, and well being of members.
- Completes paperwork as assigned in a timely manner.
- Is personally responsible and held accountable for work hours and time management as coordinated with the Program Director.
- Provides support to other staff members as needed.
- Attends agency staff meetings unless approval for non-attendance is secured by the Program Director
- Assisting with APR reports and the HUD data base system in e-snap
MINIMUM QUALIFICATIONS AND CERTIFICATIONS & LICENSES
Education, Training and Experience :
A typical way of obtaining the knowledge, skills and abilities outlined above is through graduation from high school and two (2) to four (4) years of direct care experience working in mental health.
Licenses; Certificates; Special Requirements :
- California driver’s license & current vehicle insurance / registration if driving; and,
- Reliable means of transportation capable of passing vehicle safety inspection if more than five years old excluding all modes of two-wheeled transport inclusive of bicycles, mopeds and motorcycles.
