• and lead efforts to reduce unnecessary hospitalizations through collaborative care.
• Position Summary :

We are looking for a

• Physician (MD / DO)
• to lead rounding efforts in assisted living settings throughout the Twin Cities. You'll develop care plans, engage families, and collaborate with interdisciplinary teams—all in a
• flexible, mobile, and high-touch practice environment.
• Key Responsibilities :
• Visit patients on a regular schedule in designated senior living communities
• Diagnose and manage both chronic and acute health conditions
• Support end-of-life planning and advanced directives where appropriate
• Collaborate with on-site teams to improve health outcomes and avoid unnecessary transfers
• Foster meaningful patient-provider relationships
• Qualifications :
• MN-licensed MD or DO in good standing
• Background in internal medicine, geriatrics, or family medicine
• Passion for person-centered care and team collaboration
• Reliable transportation and valid driver's license
