Overview

General Description

The purpose of this position is to provide moderately complex case management work in two identified areas within the Liaison Department : Our community subcontractors to include (Trinity Homeward Bound, El Paso Behavioral Health System, Rio Vista Behavioral Health amongst others). Work involves developing short-term contact with clients, client families, and service providers for medical, social, educational, and related service needs. The position works under general supervision with moderate latitude for the use of initiative and independent judgment.

This class works under general supervision, independently developing work methods and sequences.

Duties and Responsibilities

The functions listed below are those that represent the majority of the time spent working in this position. Management may assign additional functions related to the type of work of the position as necessary.

  • The incumbent is responsible for the receiving and internal processing of admission and discharge documentation provided by any of the subcontracted facilities the client utilizing CBCP funding receives services at.
  • The Liaison is responsible for providing authorization numbers to the subcontractors where the client has received services at; after reconciliation of that admission and discharge paperwork is tracked in spreadsheet and recorded on our recordkeeping system.
  • Responsible for receiving and processing the claims sent by subcontractor facilities, in order to process payment for services provided to the clients; the crisis community liaison will work closely with our accounting department to ensure that all processing of claims is accurate and paid on time to the subcontracted facilities.
  • Ensures completing manual entry of data into the EHR system and the CARE system accordingly.
  • Crisis Community Liaison ensures continuity of care and proper linkage to services, and clinical follow-up is conducted following client discharge.
  • Documents any service delivery, intervention conducted, communication with others, to include outside agency personnel with rationale for intervention, date, time, name of person spoken to, and purpose.
  • Provides coordination to community resources and services as clinically indicated.
  • Ensures all clients have current assessments and treatment plans as per Regulatory requirements.
  • Documents and provides a 24 hour turn around / lag time and delivery of all documentation.
  • Performs quality assurance tasks as needed.
  • Performs other duties as assigned.

Minimum Education and Experience Requirements

Requires a Bachelor's Degree. in a Social Sciences or related behavioral field. Preferred - A Master's degree in Psychology, Social Sciences or related behavioral field supplemented by two (2) years of direct casework / case management experience in a community service setting; or possession of any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities.

Experience with trauma-informed services; cognitive behavioral therapies, including Dialectical Behavioral Therapy (DBT); and motivational therapies including the use of incentives, preferred.

Required Knowledge and Abilities

Knowledge of trauma-informed theories, principles, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions), preferred.

Physical Demands

Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.

  • Special Certifications and Licenses

  • Must be able to be credentialed as a Qualified Mental Health Professional-Community Service (QMHP-CS) certification in the timeframe designated by EHN.
  • Must possess and maintain a valid state Driver's License with an acceptable driving record.
  • Must be able to pass a TB, criminal background, and drug screen.

  • Credentialed, certified, and licensed professionals with adequate training in person-centered, family centered, trauma informed, culturally competent and recovery-oriented care.
  • Acquires and maintains credentialing requirements in accordance with local, state, and federal requirements.
  • Responsible for adhering to the EHN common purpose and service framework to continuously provide exceptional care to all constituents.
