Experience :

  • Five years of progressively responsible Physical Therapy experience.
  • Minimum of two years of Physical Therapy experience in a clinical setting with a frail or elderly population.
  • Two years of supervisory experience with demonstrated ability to mentor and develop team members.

License :

  • Valid California Physical Therapy License
  • Certifications :
  • BLS
  • Must-Have :

  • Excellent leadership, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members.
  • Highly motivated and self-directed, with the ability to execute tasks in a quickly changing environment and make sound decisions in emergency situations.

  • Description :

    The Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants in the center and at home.

    This role requires close coordination with the Physician, rehabilitation staff, and other interdisciplinary team members to maximize participant independence and safety and develop or restore function.

    The Physical Therapist collaborates with the Occupational Therapist to efficiently operate the rehabilitation department.

    The Physical Therapist is a member of the management team and facilitates collaborative relationships across the organization and with external stakeholders to promote the values, team culture, and mission.

    Duties include conducting Physical Therapy assessments to determine care needs, preferences, and goals of participants.

    The Physical Therapist actively participates in interdisciplinary team meetings to develop participant care plans.

    Responsibilities include delivering and documenting Physical Therapy interventions in accordance with physician orders and care plans, including providing active and passive exercises, muscle re-education, gait, and functional training of activities of daily living.

    Supervision of Therapy Assistants includes providing training, instruction, and guidance needed to implement Functional Maintenance Programs.

    The Physical Therapist provides training and instructions to staff, participants, and caregivers / families in the use and care of therapeutic equipment.

    They are responsible for ordering, tracking, and managing maintenance for equipment.

    Staying alert to input from team members, participants, and caregivers is essential, with timely updates to the interdisciplinary team regarding any changes in participants' conditions or functional status.

    Collaboration with other team members supports smooth care transitions between various settings, including hospitals and home care.

    In coordination with the marketing team, the Physical Therapist also supports enrollment of prospective participants into the program.

    Participation in end-of-life care coordination and support is required.

    Additionally, the Physical Therapist manages direct reports, including training, supervising, and mentoring.

    Completing all required documentation in a timely and accurate manner is essential.

    Privacy protection and confidentiality of all procedures and information is a commitment.

    Adherence to organizational policies and participation in required Quality Improvement activities and staff training is expected.

    Regular communication with supervisors and the team regarding workload and priorities is important.

    Timely completion of all mandated trainings and occupational health screenings is necessary.

    Flexibility in performing assignments as business needs evolve is required.

    Other duties may be assigned as needed.
