Position Overview :
We are seeking a compassionate and dedicated Early Intervention Service Coordinator to join our team. In this crucial role, you will provide early intervention service coordination for infants, toddlers, and their families / caregivers, ensuring they access and benefit from the necessary services and supports. You will act as a key liaison between families and service providers, facilitating the delivery of services in accordance with the Individual Family Service Plan (IFSP) and applicable regulations.
Key Responsibilities :
- Service Coordination : Oversee initial and ongoing evaluations and assessments; facilitate, develop, and review IFSPs; ensure timely delivery and coordination of early intervention services.
- Family Support : Assist families in understanding their rights, making informed choices, and accessing both early intervention and community services.
- Communication : Facilitate effective communication between families and service providers; advocate for families and children as needed.
- Monitoring & Tracking : Maintain accurate records, track "at-risk" infants and toddlers, and ensure compliance with procedural safeguards.
- Transition Planning : Develop and implement transition plans for children moving to Part B or other services upon reaching age three.
- Emergency Response : Assess and respond to emergency situations, ensuring the health and safety of the child.
Qualifications :
Education : Bachelor's degree in early intervention, early childhood special education, child development, special education, family counseling, social welfare, psychology, or a related field with at least 12 college credits in relevant areas; or an Associate's degree with 60 college credits and three years of relevant experience.Experience : Minimum of one year of experience working with children, families, or individuals with disabilities; prior experience in Early Intervention services and familiarity with PELICAN-HCSIS is preferred.Clearances : Must have Act 33, Act 34, and FBI criminal clearances.License : Valid PA driver's license and access to a personal, insured vehicle.Preferred Skills :
Strong interpersonal and communication skills.Ability to work collaboratively with families, service providers, and agency staff.Experience in coordinating early intervention services and managing complex cases.Why Join Us?
