Key Responsibilities :

• Utilize your personal recovery experience to provide support.
• Educate families and communities on mental health and recovery.
• Serve as a positive role model and mentor.
• Encourage and support peers in their recovery journey.
• Link individuals to essential community resources including housing, education, and employment assistance.
• Lead recovery education sessions and activities.
• Develop individualized Wellness Recovery Action Plans (WRAP) to support personal goals.
• Facilitate engaging peer support groups.
• Offer socialization opportunities in the community.
• Reach out to individuals with high rates of inpatient hospitalizations.
• Connect with individuals resistant to engaging with the behavioral health system.
• Assist clients in creating and implementing personalized service plans.
• Support individuals during the intake process.
• Educate clients on healthcare changes and their benefits.
• Guide individuals seeking SSI / SSDI benefits through the SOAR process.
• Provide support during client discharges and conduct follow-ups as necessary.
• Empower clients to identify their strengths and resources.
• Work as part of a wraparound team, if applicable.
• Coordinate services as part of case management responsibilities.
