Schedule : Full Time, 40 hours per week

Shift : Monday Friday, 8am 5pm, some call, rotating weekends

Location : Team Green : This position will be conducting patient home visits in the Kissimmee, Celebration, St. Cloud, and Osceola County area which includes but is not limited to the following zip codes : 32836, 32821, 32830, 32837, 32824, 32827, 32832, 34773, 34771, 34772, 34769, 34744, 34743, 34741, 34746, 34748, 33896, 34759, 34747 Team Pink : This position will be conducting patient home visits in the East Orlando, South Sanford, Winter Springs, Oviedo, East Longwood, Waterford, Christmas area which includes but is not limited to the following zip codes : 32773, 32732, 32750, 32708, 32765, 32766, 32826, 32820, 32709, 32828, 32833, 32831, 32832 Team Blue : This position will be conducting patient home visits in the West Longwood, Lake Mary, Apopka, Mt Dora, Sanford area which includes but is not limited to the following zip codes : 32757, 32777, 32798, 32712, 32768, 32703, 32779, 32746, 32771 Team Yellow : This position will be conducting patient home visits in the Winter Park, Casselberry, Altamonte Springs, Maitland area which includes but is not limited to the following zip codes : 32701, 32707, 32730, 32751, 32789, 32792 Team Purple : This position will be conducting patient home visits in the Union Park, Conway, Azalea Park, Audubon Park, Vista Lakes area which includes but is not limited to the following zip codes : 32817, 32814, 32803, 32807, 32825, 32813, 32822, 32829, 32812, 32809 Team Orange : This position will be conducting patient home visits in the Downtown Orlando, Pine Hills, West Orlando, Ocoee, Winter Garden, Windemere area which includes but is not limited to the following zip codes 34760, 34787, 34734, 34786, 34761, 32818, 32835, 32819, 32811, 32808, 32810, 32714, 32804, 32805, 32839, 32806, 32801.

The role you'll contribute : Primarily responsible to carry out the treatment plan established by the Occupational Therapist for the rehabilitation of patients who have impairments, functional limitations, disabilities or changes in physical function and health status. Intervention is purposeful and skilled utilizing therapeutic exercise, psychosocial, sensorimotor and perceptual motor skills and other procedures to produce changes in the condition consistent with the diagnosis and prognosis. Interact and collaborate with the patient and family as well as the clinical team.

The value you'll bring to the team : Provides occupational therapy intervention based on the plan of care, Provides therapeutic exercises to develop strength and endurance, range of motion and flexibility; therapeutic activity to improve activities of daily living, use of adaptive equipment and adaptation of physical environment; neuromuscular re-education to improve balance, coordination, kinesthetic sense, posture, and proprioception; fine motor training, perceptual and cognitive retraining, sensorimotor activities, and other activities within the scope of practice, Provides modalities including ultrasound and electrical stimulations (if certified), hot / cold packs, as ordered, Instructs the patient and caregiver in care and use of adaptive devices and techniques and the patient's occupational therapy program including the home program, Observes and records activities and findings in the clinical record and reports to the occupational therapist and / or physician the patient's reaction to treatment and any changes in patient's condition, or when there are deviations from the plan of care.

Qualifications - External The expertise and experiences you'll need to succeed EDUCATION AND EXPERIENCE REQUIRED : Associate degree in Occupational Therapist Assistant, At least one year of experience under the supervision of a licensed Occupational Therapist EDUCATION AND EXPERIENCE PREFERRED : Prefer at least 2 years of clinical experience as a Occupational Therapist Assistant with skills and experience working with adult and geriatric rehabilitation cases, Prefer home health experience within the past 3 years and current knowledge of homecare requirements LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED : Basic Life Support - CPR Cert, Occupational Therapy Assistant This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
