We are seeking a full-time Youth Residential Licensed Practical Nurse. Licensure as an LPN is required.

The salary range for this position is $47,604.13 to $55,766.68. This position is eligible for a sign-on bonus of $3,000!!

Essential Functions :

• Provides direct care and nursing services to residents as prescribed by individual treatment plans.
• Assist with Medication Administration, medical records, medical charts, etc.
• Participates in multi-disciplinary care team meetings as needed.
• Monitors existing / chronic health conditions of residents.
• Obtains patient data and vital signs.
• Complies with all reporting requirements, biohazard program requirements, medical protocols, etc.
• Attendance at meetings per prior arrangement, and participation as an interdisciplinary team member.
• Meets billing productivity requirements established.
• Required to be on-call for emergencies at various times throughout the year.
• Other duties as assigned by the Practice Manager.

Minimum Requirements :

Must be available evenings and weekends.

Degree from an accredited LPN program.

Must be licensed in Ohio as a Licensed Practical Nurse.

Two years experience nursing experience within the medical or behavioral health field.

Broad knowledge of community service systems.

Willing to participate in and lead collaborative community-building activities.

Able to effectively communicate through verbal / written expression.

Must be able to use an electronic health record (EHR).

Must be able to operate within an Internet-based, automated office environment.
