Job Summary

We are seeking a compassionate and experience Director of Patient Care to join our Home Health Agency. The director will provide leadership and oversight of all patient care activities , ensuring that services are delivered in accordance with state, federal, and accreditation standards. This role is responsible for managing clinical staff, coordinating care, and ensuring high-quality outcomes for patients in the home setting.

The ideal candidate will bring strong leadership skills, a deep understanding of home health regulations, and a passion for patient-centered care.

Duties

• Plan, implement, and evaluate patient care services to ensure compliance with CMS, State, and agency standards
• Lead and manage the nursing team to deliver exceptional care to residents.
• Develop and implement individualized care plans based on patient assessments.
• Ensure compliance with HIPAA regulations and maintain confidentiality of medical records.
• Oversee documentation review and ensure accuracy in patient monitoring and reporting.
• Provide training and support for nursing staff on EMR systems and medical terminology.
