Summary

This position treats patients in a variety of modalities, educates the family / patient, and assures continuity of treatment. Responsible for all necessary documentation as required by accepted medical guidelines.

This position works under the supervision of the Registered Physical Therapist and is employed by Mosaic Health System.

Duties

• Treats patients using a variety of modalities and procedures under the treatment plan formulated by a Registered Physical Therapist.
• Provides patient / family education.
• Documents patients response to treatment program, communicates with physicians, Physical Therapist, and other interdisciplinary team members, and inputs charges as appropriate.
• Participates in staff meetings, family conferences, and interdisciplinary team meetings.
• Participates in continual process improvement.
