Sunbelt Staffing is seeking a skilled Home Health RN for a contract assignment in Annapolis, MD. This role involves providing patient-centered care in the home setting, utilizing OASIS for documentation and Homecare Homebase (HCHB) for case management. The ideal candidate is independent, compassionate, and able to deliver high-quality care while managing a flexible caseload.

Key Responsibilities :

Conduct comprehensive patient assessments and home visits

Complete admissions, resumptions, recerts, and discharges using OASIS

Document care and update plans in Homecare Homebase (HCHB)

Educate patients and caregivers on treatment plans and disease management

Collaborate with interdisciplinary healthcare team for coordinated care

Qualifications :

Active RN license in Maryland

Minimum 2 years of nursing experience, with at least 1 year in home health

Required proficiency in OASIS and Homecare Homebase (HCHB)

Reliable transportation, drivers license, and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
