This role is ideal for someone passionate about delivering exceptional an client experience.

Key Responsibilities

Care Management :

• Oversee the ongoing management of client services, ensuring their needs and expectations are consistently met.
• Act as the main point of contact for clients and their families, addressing concerns and resolving issues with empathy and efficiency.
• Conduct assessments and reassesments for new clients and assist in developing personalized care plans.
• Maintain detailed records and communication with caregivers and internal team to deliver seamless care.
• Recommend and support the implementation of caregiver training initiatives, including specialized Alzheimer’s / Dementia care programs.
