POSITION SUMMARY : The Community Relations Liaison, in conjunction with the Admission Team, promotes the organization's services to referral sources, patients, families, and the community. The role involves educating patients and families about care plans and ensuring high-quality care.
REPORTS TO : Director of Operations or Executive Director.
QUALIFICATIONS :
- Education : Bachelor's degree in a healthcare-related field preferred.
- Experience : 2+ years in hospice, palliative, home health, or similar fields preferred.
- Core Competencies : Knowledge of Medicare, Medicaid, and private insurance regulations; excellent communication skills; ability to observe and report patient status; experience in marketing strategies preferred.
- Other : Valid driver's license and auto insurance.
FUNCTIONS & RESPONSIBILITIES :
Collaborate with the admission team.Establish presence within the community and with facility staff.Assist with admission documentation and patient information.Respond to referrals and explain programs to stakeholders.Develop marketing plans with referral sources.Participate in staff meetings and complete documentation accurately.Comply with policies, laws, and standards.Perform other duties as assigned.OTHER DETAILS :
