Mandatory Job Requirements :
- Board Certification : Board Certified or Board Eligible within 3 years of Residency in Pediatrics - REQUIRED
- State License : Active OR License - REQUIRED
- Background Check : Clean Malpractice / Background - REQUIRED
- Availability : Must be available at the time of name clear (must note if available with the specified dates) - REQUIRED
- DEA : Required during Credentialing
