Job description41,600 / Annually Primary Role : This position manages all household activities at one assigned home for Lehigh Valley Human Services. The House Supervisor is responsible for assisting the Program Manager with budget management, developing schedules, maintaining records, and managing processes in the assigned home to ensure 6400 regulations are being adhered to. Everyday Lives : Values in Action : Assist and support individuals to have valued social roles such as family member, friend, neighbor, student and employee. Support each person to become more independent, make decisions and choices, and live a meaningful dignified life. Advocate on behalf of the individual and family using person centered language. Encourage each person served to self-advocate. Responsible for all supervisory activities for a single assigned home. Provides operational oversight and supervision of assigned home in the absence of the Program Manager. Ensures compliance with all regulating agencies and internal policy and procedures. Works directly with individuals for a set number of hours per week in program hours (based on total number of homes assigned). Works collaboratively with all interdisciplinary team members to ensure ISPs for individuals are completed / updated as necessary; and ensures any resources needed for implementation are provided. Sets up and oversees grocery store food accounts, grocery shopping, menu development, individual money accounts, nightly MAR audits and house petty cash. Evaluates program resources and identifies areas of strength and weakness. Participate in Incident Management Process as assigned. In emergency situations, provide necessary staffing coverage in program / home to maintain safety and welfare of program participants / residents. Participates in transporting individuals to doctor appointments and acts as an advocate with medical personnel. After the appointment, provides necessary follow-up to program staff. Audits cleanliness of the home and checks all food label expiration and discards items nightly. Facilitates and ensures all report deadlines are met. Practices safe and sound crisis prevention and intervention strategies by promoting and teaching the use of preventative strategies, therapeutic activities and recreation, and early intervention techniques. Promotes a safe and caring environment where individuals can learn and grow while enjoying a healthy and productive lifestyle. Understands internal and external policies and follows employee rules of conduct and performance as set forth by LVHS Promotes community awareness by demonstrating responsible, courteous behavior and a positive perception of individuals with developmental disabilities. Conduct and Supervise fire and safety drills. Work Environment Office environment combined with Program environment, which includes community and recreational activities. May experience exposure to body fluids and the need to provide personal, private care to individuals. Varied shift hours will be required. Driving and a valid Driver's license is required. Such information may include, but is not limited to identifying information, medications, treatment plans and procedures, physician’s and nurses’ notes and orders, medical and other health related records, medical and other health related appointments. Federal and state laws and regulations protect the release of such information. May share protected health information verbally, in writing, or electronically with medical and nursing staff, co-workers, staff of collateral agencies and family members, given the required authorization. All use of protected health information, verbally or in any written or electronic form, shall be used or shared only to the extent required to perform treatment responsibilities, produce required documentation for payment, and assure necessary health care. All protected health information, in any written or electronic medium, shall be kept secure as detailed in LVHS’s Protected Health Information policy. General Professional confidentiality is expected at all times both within the company and withother agencies. Other Skills Perform documentation with accuracy, legibly and in a timely manner consistent with high professional, ethical standards. Ability to read, write and speak the English language in order to perform job duties. Follow written and / or verbal instructions. Perform basic Mathematical functions such as addition, subtraction, multiplication and division. Manage multiple tasks. Proficiency in Microsoft word, Excel, and Outlook. Complete other duties as assigned. Two years' experience working with special populations in a 6400-group home required. Must have a High School Diploma or equivalent. Must demonstrate the ability to communicate both orally and in writing and to be able to read, interpret and apply written information. Must have the ability to successfully complete required Medication Administration, First Aid and CPR certifications and other trainings mandated by regulation within specified timeframes. Must be at least 18 years of age. Position will demand frequent bending, standing, walking, sitting, reaching, lifting or carrying, and speaking on the phone. Must be able to participate in activities of daily living and ambulate throughout various homes and out in the community, including steps. Must be able to lift a minimum of 25 lbs. Individuals receiving services, their family members, supervisors, and other Direct Support Staff. Perform other duties and responsibilities as required. Physical Abilities Must be able to lift and transfer 50 pounds using proper body mechanics, climb and descend 20 stairs carrying 10 pounds, bend to retrieve an object at floor level 6-12 times hourly, crouch at the knees 6-12 times hourly, kneel on both knees, drive for a period of one hour 3- 6 times daily, stand for a period of one hour 6-12 times daily, sit for a period of one hour 6-12 times daily, push / pull 10 pounds, grasp an object with at least one hand, must have at least 20 / 40 combined vision with or without corrective lenses and must be able to hear a normal spoken voice with or without hearing assistance Confidentiality The Health Insurance Portability and Accountability Act (HIPPA) apply to any access to the protected health information of the people in care.
