Care Coordinator

Overview

A well respected hospital in Manhattan is seeking a skilled Care Coordinator to support patients and clinical teams through in-person coordination and digital workflow support. This role blends onsite patient interaction with telehealth-related responsibilities, ensuring patients receive timely guidance, follow-up, and seamless care navigation.

Responsibilities :

• Serve as an in-person point of contact for patient needs, inquiries, and daily care coordination
• Support virtual visit scheduling, provider communication, and follow-up outreach
• Assist patients with navigation of digital care platforms and remote instructions
• Contribute to the creation, evaluation, and refinement of telehealth policies, workflows, and protocols
• Analyze telehealth-related quality, outcomes, and data to support ongoing improvement
• Monitor patient status during virtual encounters, recognizing clinical or behavioral changes requiring escalation
• Collaborate closely with interdisciplinary teams to support safe and effective telehealth service delivery
• Document all interactions accurately in the EHR and maintain strict adherence to HIPAA and facility standards

Qualifications :

• Associates degree in a health related field required; Bachelors degree preferred
• At least 3 years of direct patient care experience required
• Experience with EHR systems, telehealth platforms, or remote monitoring tools
• Ability to work every other weekend and selected holidays
• Organized, adaptable, and able to support both onsite and virtual care needs
