A company is looking for a Family Nurse Practitioner (Virtual - Illinois Licensed).
Key Responsibilities
Conduct virtual wellness visits and health assessments for patients
Collaborate with clinical in-home team members to provide comprehensive care
Perform thorough health risk assessments and medication reviews
Required Qualifications and Education
Board Certified as a Family Nurse Practitioner in Illinois
Active Family Nurse Practitioner License in Illinois
Exhibit high levels of professionalism, integrity, and compassion
Technologically savvy and comfortable using digital tools for patient care
Ability to work independently or in a team environment
