About The Position

Job Title : Clinical Care Coordinator

Profession : Nursing

Experience : Minimum of 2 years of experience in acute care coordination

License : Required

Certifications : BLS

Must-Have :

Experience with Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs

Experience with discharge planning and utilization review

Minimum of 3 years hospital experience

Description :

Profiles go directly to the unit.

Two supervisory references are required at the time of submission; one within 12 months and one within 3 years of employment.

Candidates may not have been directly employed by any facility associated with the health system within the past year.

Candidates employed PRN through outside firms will be considered.

Travelers will work two of the three major holidays (Thanksgiving, Christmas, and New Year's).

Managers will make the final scheduling decisions.

Holiday time off will be considered, but not guaranteed.

Previous experience in acute care coordination is mandatory.

The patient assignment ratio is 1 : 20.

A clean, professional appearance is required—business casual conservative dress with no open-toed shoes is mandatory.

Minimal to no perfume is permitted.

Candidates must be fast learners, adaptable, and flexible.

Excellent attendance is expected, and candidates must take lunches and breaks.

The hospital may cancel up to 4 shifts during the 13-week assignment.

Charting system is Cerner.

Candidates must be able to cover all adult areas of the hospital.

If no experience in women's health, the traveler will not cover this area.

Shifts are 8 hours long with a preference for minimal time off requests.

A 12-hour shift option is available if the candidate has a background in the emergency department and is willing to work night shifts.
