• Acute care experience

• Experience with HMOs, IPAs, and advance directives
• Strong patient advocacy and care coordination skills
• Ability to float between units based on need

Description :

This role requires a dedicated Case Manager with experience in acute care settings.

The ideal candidate will have a strong background in case management.

Candidates must be available to work every other weekend.

Float requirements may apply based on departmental needs.

We accept first-time travelers with clinical approval.

Strong communication and collaboration skills are essential.

Knowledge of discharge planning and care coordination processes is crucial.

Candidates will manage a caseload of up to 25 patients.

Experience with Epic charting is required.

This position involves interacting with diverse patient age groups, including adolescents, adults, pediatrics, and geriatrics.

Candidates must be familiar with the Centers for Medicare and Medicaid Services guidelines and HIPAA regulations.

Commitment to working three of the five major holidays is expected.

References covering recent employment in similar settings are required.

All travelers should be open to floating between facilities as needed.
