About The Position

Job Title : Registered Nurse I

Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience, depending on the specialty.

License : Must have license in hand.

Certifications : CPR, BLS

Must-Have : Must be proficient in OASIS and HomeCare HomeBase EMR and have used them within the last 6 months.

Description :

Provides coordinated and directed home nursing care according to physician's orders based on agency policies and procedures.

Competently applies the nursing process in all activities.

Responsible to the Clinical Manager.

Directly supervises and evaluates home health aides and licensed practical nurses / licensed vocational nurses.

Must have reliable transportation and a valid driver’s license.

Agency-required liability insurance is mandatory.

Job entails home visits and periodic office meetings.

Average number of patients per day : 5-8.

Weekly point system requires a minimum of 30 points, with each visit and activity having different point values.

Contractor will be assigned 5-10 patients per day, equating to roughly 6-8 points.

Types of cases include start of care, wound care, and follow-up visits.

Weekend requirements vary by location and profession.

On-call responsibilities vary, approximately one weekend per month.

Must be open to traveling up to 75 miles from the designated care center.

Expected weekly travel mileage is approximately 200 miles.

Documentation must be completed within 24 hours of patient appointments.

All documentation that impacts billing and patient care must be timely and accurate.

No guaranteed hours with this contract; must achieve productivity points to maintain employment.

Must be willing to accept all assigned patients, regardless of location.

This position may involve occasional visits to assisted living facilities.

Must be open to on-call rotations.

Local candidates are accepted at the same rate.

Recent experience in specialty is required.
