HHH Social Worker

Position Summary

/ Career Interest:The Social Worker is expected to support the mission, vision, and values of Olathe Medical Center, Inc. This position is responsible for consultation and direction of social services to meet the needs of Hospice and/or Home Health patients. He/She will assure that social service/counseling services are integrated and coordinated with other team members. He/She will uphold the philosophy of the Hospice Agency in meeting comfort care needs of the patient and family.Responsibilities and Essential Job Functions

• Demonstrates appropriate assessment and intervention skills in meeting the psychosocial needs of Home Health and Hospice clients and families.
• Documents all assessments, interventions, and contacts.
• Demonstrates awareness and competence in working with clients and families within the context of disability, age, ethnicity, and culture.
• Performs bereavement assessment and counseling per protocol to hospice families/S.O. after patient death.
• Offers bereavement counseling to community members on an as needed basis.
• Facilitates bereavement groups as scheduled
• Attends and participates in IDT meetings.
• Communicates appropriately and effectively with clinical and office staff in meeting client needs.
• Identifies self and other staff learning needs and participates in professional growth, i.e. teaching, education and professional associations.
• Demonstrates the knowledge and ability to utilize community resources to meet the needs of the Hospice and Home Health patients and families.
• Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience

• Masters Social Work
• 2 or more years in Social Worker Related Field, preferably Community-Based Services and/or Healthcare Setting

Preferred Education and Experience

• 3 or more years of Experience

Required Licensure and Certification

• Licensed Masters Social Worker(LMSW) - State Board of Behavioral Sciences upon Hire
• CPR/ AED/ BLS - Other BLS within 14 days
