Job description

A reputable organization in Queens, NY is seeking a Client Care Coordinator to join their team at a renowned family shelter. This position offers an opportunity to make a meaningful impact by providing top-tier case management services to families in need.

About the Opportunity :

  • Hybrid or Remote : On-site
  • Shift : Full-time
  • Schedule : Monday to Friday, 9am-5pm - All shifts have a late night of 1pm to 9pm, and the rest of the days are 9am to 5pm.
  • Hours : 35 hours / week
  • Setting : Family Shelter

Responsibilities :

The Client Care Coordinator will be responsible for :

  • Managing a caseload of approximately 50 families
  • Completing biopsychosocial evaluations within 30 days of client entry
  • Conducting weekly individual sessions with clients
  • Leading "Ready Set Goal" group sessions for young parents
  • Creating safety plans for domestic violence situations

    • Qualifications :

  • Master's in Social Work required
  • LMSW within one year of hire
  • Bilingual Spanish
  • Experience with biopsychosocial assessments preferred
  • Ability to complete pre-employment clearances
  • TB test, First Aid / CPR, and overdose prevention certifications
  • Strong interpersonal skills
  • Excellent communication skills
  • Organizational skills

    • Desired Skills :

  • 1+ years of experience in social work
  • Master's degree in Social Work
  • LMSW certification
  • Biopsychosocial assessment experience
  • Proficiency in Spanish
