ID#36140A - Provide supportive counseling and case management services within the scope of practice, utilizing both individual and group modalities. Deliver crisis intervention services to address immediate concerns and risks, offer initial support and assistance, and foster a therapeutic relationship between the client and the clinic.
Key Responsibilities
Maintain a caseload of at least 50 clients.
Provide Officer of the Day coverage for crisis and walk-in clients as scheduled.
Screen clients for intake appointments and / or referrals.
Participate as an active member of a multidisciplinary treatment team.
Attend all scheduled clinical debriefings, staff meetings, and team meetings to assess client needs, strengths, and resources, and contribute to treatment planning.
Complete all required documentation—including charts, reports, service plans, needs assessments, and correspondence—within agency and Department of Mental Health / Medicare guidelines and timelines.
Collaborate professionally and effectively with clinical and psychiatric staff.
Network with community resources to facilitate client linkage and referrals.
Conduct home visits when necessary and safe.
Work at least one evening per week.
Perform other duties as assigned by the Program Manager.
