Community Healthcare Network is seeking a Full-Time Social Worker / Healthy Steps Program who will work along with and in support of pediatricians and in partnership with the clinical and the administrative team.
The Social Worker provides support for families with children age 0-5 years and provides caregiver support to facilitate the development of attachment, self-regulation skills, and family resiliency through one-on-one and group interactions.
JOB FUNCTION :
Responsibilities include, but are not limited to :
- Conduct in-office or remote visits with parents, caregivers and their young children, either jointly with the physician / nurse practitioner or alone.
- Work closely with providers around care coordination, goal setting, counseling, coaching and education about key aspects of a child’s development.
- Closely track progress made in ongoing checkups that look at both child development and family factors.
- Provide anticipatory guidance to parents and other primary care providers around common concerns such as early learning, sleep, feeding, discipline, toilet training, etc.
- Provide referrals and track follow-up, as appropriate, to help families make connections to key resources within the community.
- Work closely with the pediatric team as a resource for developmental and parenting concerns.
- Facilitate parents’ / caregivers’ education groups.
- Conduct one-on-one therapy visits on a weekly basis.
WHAT WE LOOK FOR :
- Master’s degree in social work required.
- Currently licensed and registered to practice social work in New York State or must obtain license and be registered to practice social work in New York State within six (6) months of hire.
- Minimum 2 years’ of experience with parenting, child development, social work, mental health counseling, psychology, early childhood education, or early intervention is preferred.
