A Brief Overview

Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves a resource to others in work-team and department.

Position Details and Preferred Qualifications

Post-Master's experience is strongly preferred. Medical Social Work experience is also preferred. This is an outpatient, Seidman Cancer Center position within the Head & Neck Clinic. It is a very high needs and complex clinic, which assists multiple providers and follows an average of 300 patients . The Social Worker will be providing supportive care to patients and families, referrals to community resources, and identifying and addressing barriers to oncology care, and will also be part of a multidisciplinary team of physicians, nurses, physical therapy, occupational therapy, and other team members as needed. The schedule is Monday-Friday.
