SUMMARY OF RESPONSIBILITIES :
The Resident Care Coordinator (LPN) provides clinical leadership for nursing staff in the ALF setting and ensures smooth resident care operations. Responsible for providing nursing care within the Missouri Nurse Practice Act, ensuring resident well-being through quality care aligned with assisted living philosophy and state regulations.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Accepts / gives shift report; makes rounds to assess residents and needs.
Inspects environment; identifies and requests corrections.
Plans / delegates assignments; ensures equipment / supply levels.
Identifies clinical / operational problems; assists in emergencies.
Communicates needs to Director of Resident Care / House Supervisor.
Performs CNA / CMT evaluations; participates in disciplinary actions.
Reports / documents incidents / accidents and assists in investigations.
Ensures care meets regulatory requirements; participates in committees.
Completes records / reports; participates in assessments and care plans.
Reviews / transcribes / initiates physician orders.
Coordinates admissions, transfers, discharges.
Supervises staff, provides direct care, and oversees documentation.
Communicates with residents, families, and physicians.
Counts narcotics; administers medications / treatments.
Implements / audits infection control; prepares staff / rooms for new residents.
Provides staff instruction; ensures staff attend in-services / meetings.
Attends continuing education; promotes positive relations and LTC image.
Ensures timely handling of complaints; maintains department relationships.
Assists in marketing plan; supports Mission, Standards, Quality Improvement.
Adheres to Ethical Standards, HIPAA, and regulatory / licensure requirements.
Maintains regular, reliable attendance.
OTHER DUTIES
Performs other duties as assigned.
QUALIFICATIONS
LPN with current Missouri license.
One year nursing experience preferred; gerontology / LTC beneficial.
Graduate of accredited school awaiting State Board Exam.
Completion of Missouri ALF Assessor Training required.
Strong interpersonal, leadership, organizational, and time-management skills.
