We are seeking a specialized, outcomes-focused Nurse Practitioner to join our team dedicated to revolutionizing wound care. If you are passionate about providing the highest level of evidence-based wound treatment outside of a traditional clinic setting, this is your opportunity. You will function as the primary clinical expert, traveling to patient homes and skilled facilities in Macomb County to manage complex, acute, and chronic wounds.
The Role : Core Responsibilities
Direct Clinical Care : Perform comprehensive physical assessments, accurate diagnoses, and implement advanced, evidence-based treatment plans.
Specialized Procedures : Execute wound care procedures including debridement, tissue biopsy, and selecting specialized dressings based on patient needs.
Documentation & Compliance : Maintain meticulous Electronic Medical Record (EMR) documentation for all encounters, ensuring regulatory compliance and seamless record sharing.
Care Coordination : Act as the primary wound care resource, coordinating care adjustments with referring physicians, nurses, and allied health professionals.
Patient Education : Empower patients and their families through thorough, easy-to-understand education on prevention and long-term wound management.

Qualifications & Experience
Minimum Requirements :
MSN or DNP from an accredited institution.
Current, unrestricted NP License in the State of Michigan.
A minimum of two years of experience as a practicing Nurse Practitioner.
Proven capacity for independence, organization, and clear communication.
Valid driver’s license and dependable vehicle.
Highly Desired :
Previous experience in wound care, home health, or a geriatric setting.
Wound Care Certification (WCC, CWS, CWCN) is strongly preferred, or a commitment to obtain certification within your first year.
