Schedule : Full-time, Part-Time day / nights & PRN 12-Hour Shifts (6 : 00 - 6 : 30 w / 30 min lunch) & Evening Shifts (6 : 00 PM - 10 : 00 PM)

Position Overview

As an LPN, you will provide skilled nursing care under the direction of a Registered Nurse (RN) or physician. You will play a vital role in monitoring resident health, administering medications, and providing compassionate support to residents and their families.

Key Responsibilities

• Administer medications and treatments according to physician orders and facility policy.
• Monitor residents' health, including vital signs and changes in condition, and promptly report concerns to the RN or physician.
• Assist with resident admissions, transfers, and discharges.
• Ensure accurate and timely documentation in medical records.
• Support CNAs and other team members by providing guidance and assistance.
• Promote and protect resident rights, dignity, and safety at all times.
• Collaborate with interdisciplinary team members to provide holistic resident care.
