Job descriptionDocument visits in our quick EMR (no OASIS required) Communicate progress and discharge planning with patients and families Treat patients primarily within your designated geographic area Competitive salary : $85k–$95k + generous PTO Medical, dental, life & LTD insurance 401(k) with company match Tuition reimbursement & CEU opportunities $1,500 annual stipend for specialty certifications (Schroth, Pelvic Floor, OCS, PCS, NGS, WCS) Flexible scheduling – see patients any 7 days / week, with preference in your primary area Supportive mentorship for documentation and compliance NYS Occupational Therapy License (required) Homecare experience preferred (mentorship & training provided if new to home health) Compassionate, patient-centered care and strong communication skills

What You’ll Do :
- Provide one-on-one occupational therapy in the home setting
Develop and update individualized plans of care with patients, families, and physicians
