Job Summary :
The TPC Clinical Therapist will provide outpatient psychotherapy at The Providence Center or telehealth.
Duties and Responsibilities :
Perform a comprehensive psychosocial assessment of assigned consumers. Complete crisis assessments, initial assessments, and integrated summary forms.
Develop and implement a plan of care with ongoing consumer input. Provide individual mental health and / or substance abuse treatment and crisis assessment.
Perform record keeping in accordance with Health Information Services and other Canter requirements.
Requirements :
Master痴 degree in Social Work or related field (LCSW LMHC LFMT) or independent license required (LICSW and / or LMHC).
Supervision towards independent license available.
