Responsibilities

• Review assessment referrals received and collaborate with RN to assess patient needs.
• Assist with coordination of patient care.
• Maintain patient records, updating databases, and ensuring accurate documentation of patient information.

Job Details

Behavioral Health Connections (BHC) in Dunwoody, GA is seeking a dynamic and talented LPN Referral Management PRN to work 10-12 hours varied shifts that will review assessment referrals and work closely with the RN and other clinicians to determine the appropriate placement of patients seeking treatment. Assist the call center specialist with other duties to enhance the referral management process.

Qualifications

Nursing Degree, preferred Bachelors or Masters

Licensed LPN in current State or compact license

Current CPR, if required by a customer hospital. (Customer hospital means the hospital to which BHC is contracted to provide mobile assessment services.)

Preferred 1-3 years related work experience in Behavioral Health
